Why are there two techniques for gynecomastia?

Gynecomastia — male chest enlargement — is made of two tissues in varying proportion: fat, which is soft and diffuse and responds to liposuction, and glandular breast tissue, which is firm, sits directly beneath the nipple and does not respond to liposuction at all. Choosing one technique for a two-tissue problem is the commonest reason for a disappointing result: liposuction alone leaves a firm disc under the nipple, excision alone leaves the surrounding fullness. What does liposuction do — and what can it not do?

VASER liposuction uses ultrasound energy to loosen fat before it is suctioned, which is gentler on the surrounding tissue than conventional liposuction and lets the surgeon contour the chest edge, the outer fullness and the transition to the armpit and abdomen. In gynecomastia it does three things: reduces the overall volume, sculpts the shape so the chest reads as flat rather than merely smaller, and — used at the end — feathers the area around the excision so there is no step. What it cannot do is remove gland. Suction pulls fat through a cannula; dense glandular tissue does not pass, and a surgeon who tries to “suction the gland” leaves it in place under a smaller chest. Men with pure fatty enlargement — pseudogynecomastia — are the minority for whom liposuction alone is the whole answer; the same VASER approach is used for suprapubic fat removal.

What does gland excision do — and what can it not do?

Gland excision removes the firm disc of breast tissue through a short incision along the lower edge of the areola, where the colour change hides the scar. It is the only way to remove gland, and in a man whose enlargement is mostly glandular — the classic puffy nipple with a hard lump beneath — it is the part of the operation that makes the difference. What it cannot do is shape: excision alone in a chest with fatty fullness leaves the fullness, and sometimes a hollow where the gland was against the surrounding fat that remains. Why do most cases need both?

Because most chests have both tissues, and because the two techniques cover each other's weaknesses. Liposuction first defines the contour and reduces the fat around the gland; excision then removes the disc through a smaller incision than would otherwise be needed, since the fat no longer obstructs it; liposuction last feathers the edges so the excision site blends. A chest treated this way looks flat and natural rather than deflated, and the combination is why our surgeons in Busan plan it as the default rather than the upgrade. The exceptions are real but few: pure fatty enlargement, where liposuction alone suffices, and the rare small, purely glandular case in a lean man, where excision alone may.

Grades: how the surgeon reads your chest

• 1 Grade 1. Small enlargement, mostly gland, concentrated beneath the areola. Little or no excess skin. Excision does the most work; liposuction feathers.
• 2 Grade 2. Moderate enlargement extending beyond the areola, mixed fat and gland. The classic case for the combined technique. No or mild skin excess.
• 3 Grade 3. Larger enlargement with skin excess and sometimes a fold. Combined technique plus a judgement about whether skin will retract — the surgeon discusses this honestly, since skin removal leaves larger scars.

Scars: where does each technique leave them?

Liposuction leaves small entry-point marks — a few millimetres each — placed at the chest edge or in the armpit fold where they fade to near-invisibility. Gland excision leaves a short curved scar along the lower edge of the areola, hidden by the colour change; it matures over months and in most men becomes difficult to find. Skin removal in a high-grade case is the exception that produces a longer, visible scar, and it is the reason surgeons are conservative about removing skin at the first operation and prefer to let it retract. The honest question to ask any clinic is to see photographs of the areolar scar at six and twelve months in their own patients.

Recovery, compression and the trip to Busan

The operation is performed under anaesthesia with same-day discharge in most cases, with the surgeon explaining each step in English. A compression vest is worn for several weeks — it controls swelling and helps the skin settle onto the new contour — and it is the single most important thing a patient does for his own result. Desk work resumes within days, gym after about four weeks as the surgeon instructs, and the final shape settles over three to six months as swelling resolves.
