Day 0: the first afternoon and night after circumcision in Busan

After laser circumcision at Urogyn Men's Clinic in Busan Seomyeon you leave within the hour with a light dressing, the local anaesthetic working, and a written plan. The afternoon is rest in a hotel within a five-minute walk; the anaesthetic wears off over a few hours and the first night is the uncomfortable one — a burning ache at the suture line and, for most men, one or more night erections that pull at it. It is the night men are least prepared for and the reason the clinic advises two nights in Seomyeon before flying, not one.

Days 1–7: dressing, showering, night erections

• 1 Day 1 — the dressing comes off. Usually in the morning shower: let warm water soften it, remove gently, pat dry. The line looks raw and the glans, newly exposed, feels tender. Apply the ointment the urologist gave, as directed.
• 2 Days 1–3 — swelling and colour. The shaft skin below the line swells, sometimes markedly, and the line itself looks purple or yellow. This is the peak; it does not mean something has gone wrong.
• 3 Days 3–7 — night erections continue. They are the most-reported discomfort of the week and they lessen as the line heals. Emptying the bladder before bed, avoiding a full bladder overnight, and side-sleeping reduce them.
• 4 Showering daily. Warm water, no scrubbing, pat dry, ointment. No bath, pool or sea until the line has sealed — usually two to three weeks.

By the end of the first week the burning ache has faded to tenderness and the swelling has begun to settle. Most men are at a desk from day two or three; the clinic's message thread is where the day-three photograph goes if the swelling worries you, and the urologist who operated answers it.

Weeks 2–3: sutures and swelling

The absorbable sutures placed with the laser technique begin to loosen and fall away from around ten days, and are usually gone by three weeks — nothing needs removing, which is one reason our urologists prefer laser with hand suturing over staples, as the comparison guide explains. Swelling settles unevenly: a ridge of firmer, puffier skin just below the line often persists into week three and beyond, and it is lymphatic swelling, not infection. The glans, exposed for the first time, is hypersensitive to clothing for the first two weeks and then adapts; loose underwear and, for some men, a soft gauze over the glans for the first fortnight make it tolerable. Night erections are markedly less troublesome by week two.

Weeks 4–6: sex and the settled line

Sexual activity, including masturbation, waits until the urologist confirms the line has matured — usually around four weeks for basic circumcision and up to six for volume-up or extension, and later if any part of the line has been slow to seal. Starting earlier risks splitting the line, which sets healing back by weeks. When you do resume, use lubricant, go gently, and expect the glans to feel more sensitive than before for the first several encounters; that settles. By six weeks the line is a fine pale scar, the swelling is largely gone, and the result is what it will be — with the scar continuing to fade over the following year.

Flying home from Busan after circumcision

Two nights in Seomyeon before any flight; three for the extension option. A flight on day one is a night erection in an aisle seat with no way to walk it off. From day two, short-haul is fine with loose trousers, snug underwear, an aisle seat and pain relief taken before boarding; long-haul is better from day three, with hourly walking and compression stockings as after any procedure. Nothing about the flight affects the healing; it only affects how comfortable the healing is.

Work and exercise

Warning signs, plainly

• Bleeding that does not stop with ten minutes of firm, gentle pressure — especially after a night erection in the first days. Message with a photograph; come in during opening hours.
• Fever, spreading redness, warmth, pus, or a foul smell at the line — infection. Uncommon after laser; treated promptly with antibiotics.
• A gap opening in the line — dehiscence, usually after early sexual activity or a hard night erection. Often heals without re-suturing; the urologist judges from a photograph.
• Inability to pass urine, or a stream that has become a dribble — rare, assessed the same day.
• Swelling that is increasing rather than settling after day three — a haematoma or infection; message at once.

Long-term: sensation and appearance

Two honest points. The glans, protected by foreskin for a lifetime, is hypersensitive for weeks and then becomes less sensitive to touch than it was in the first days — most men describe sensation as different rather than reduced once it has settled, and the published evidence on sexual satisfaction after adult circumcision is reassuring but not unanimous; the urologist says this before surgery. And the scar is permanent: a fine pale line that fades over a year, more visible in some men than others, with volume-up and extension leaving their own characteristic appearance.
