How do UroLift and Rezum each work?
UroLift places small permanent implants that pull the enlarged prostate lobes apart and hold the urethra open — nothing is cut or heated, and the effect is immediate. Rezum injects water vapour into the prostate tissue; the steam's energy destroys the treated cells, which the body reabsorbs over the following weeks, so the gland shrinks and the channel opens gradually. Prostate size: the first filter at our Busan clinic
Size is measured on the ultrasound in the ₩600,000 full prostate check-up, and it is the first thing the urologist looks at. UroLift is best established for small-to-moderate glands — see who it suits; as prostates get larger, more implants are needed and the mechanical hold becomes less reliable. Rezum handles a wider size range, including glands too large for UroLift to hold open comfortably, because shrinking tissue scales more gracefully than retracting it. Above a certain volume, neither is ideal and the urologist will say so — the prostate hub is candid about the point at which a hospital procedure is the honest recommendation.
The median lobe: the second filter
Some prostates have a median lobe — a central portion that pushes up into the bladder outlet like a ball valve. It is the anatomy that most changes the decision. UroLift was designed for the lateral lobes, and although newer techniques can address a median lobe, a prominent one is where UroLift is least predictable. Rezum can treat the median lobe directly with an additional injection, which is why a median lobe on ultrasound often tips the recommendation toward Rezum at our Busan clinic. If your imaging from home already shows one, send it before you travel; it shortens the conversation considerably.

This is the question men in their fifties and sixties ask first, and it deserves a precise answer. Traditional BPH surgery frequently causes retrograde ejaculation — semen entering the bladder rather than leaving the body. UroLift has the strongest record of preserving both erectile function and antegrade ejaculation, because no tissue is removed or heated near the ejaculatory ducts. Rezum also preserves erectile function and preserves ejaculation in most men, but the rate of ejaculatory change, while low, is not zero and is higher than with UroLift. For a man for whom ejaculation preservation is the overriding priority, and whose anatomy permits it, UroLift is usually the recommendation; for most others, the difference is small enough that size and median lobe decide.
Speed of relief and recovery: which is faster?
UroLift is faster on both counts. The mechanical effect is immediate, most men leave without a catheter, and symptoms improve within days. Rezum works by tissue shrinkage, so the treated prostate swells first and shrinks later: symptoms can be temporarily worse for one to two weeks, a catheter is commonly needed for several days, and the full benefit arrives over one to three months. Neither is painful in the sense that traditional surgery is, and both are done by the same urologist under local anaesthesia with sedation at our Busan clinic with same-day discharge. Catheter, medication and the trip to Busan
For an international patient the practical difference is the catheter. A UroLift patient can typically fly home within a few days. A Rezum patient should plan to stay until the catheter is removed — usually several days — and to keep taking BPH medication for a short period afterwards while the effect develops, exactly as the urologist instructs. How long does each result last?
Both are durable but not identical. UroLift implants are permanent, and published follow-up data extends over several years with a proportion of men needing further treatment as the prostate continues to grow; Rezum removes tissue, and its follow-up data also extends over several years with a similarly modest retreatment rate. Neither stops BPH being progressive, which is why PSA and volume monitoring continues afterwards. What a urologist weighs is that UroLift leaves all options open — including a later Rezum or a hospital procedure — whereas Rezum's tissue effect is already done. For younger men with a smaller gland who value keeping every door open, that often favours UroLift; for men whose gland is already larger, Rezum's more durable volume reduction often favours Rezum.
Price: what the check-up decides
Both are listed from ₩10,000,000 at Urogyn, deliberately, so that the decision is clinical. What sets the final figure is the check-up: prostate volume determines how many UroLift implants or how many Rezum injections your gland needs, and the urologist confirms the exact price in writing after the ultrasound, before anything is scheduled. - Can I have Rezum after UroLift, or the reverse? UroLift followed later by Rezum is possible, because the implants do not prevent vapour treatment. Rezum followed by UroLift is less common, since the shrunken tissue changes the anatomy the implants rely on. This asymmetry is one reason younger men with smaller glands are often steered toward UroLift first.
- Will I still need tablets afterwards? Many men stop BPH medication after either procedure. After Rezum the urologist usually keeps you on it for a short period while the effect develops; after UroLift it is often stopped sooner. The instruction is individual and given in writing.
