What is phimosis in adults?

Phimosis is a foreskin that cannot be retracted over the glans, or retracts only partly or with pain, in a man past puberty. It ranges from a mild tightness noticed only during erection to a pinhole opening that obstructs urine. Causes in adults include scarring from repeated infection, a skin condition called balanitis xerotica obliterans, diabetes, and simple lifelong tightness that was never addressed. Grading the tightness

• 1 Grade 0–1 — fully or almost fully retractable. Slight tightness at the tip on erection, no pain, no infections. Usually needs nothing, or stretching alone.
• 2 Grade 2 — partial retraction. Glans partly exposed; tight ring visible; some discomfort on erection or sex. The grade at which cream and stretching are most likely to succeed.
• 3 Grade 3 — retraction only when flaccid. Cannot retract when erect; sex uncomfortable or impossible; smegma or infections recur. Cream sometimes helps; circumcision usually the reliable answer.
• 4 Grade 4 — no retraction, pinhole opening. Ballooning on urination, weak stream, recurrent infection. Circumcision; cream is unlikely to succeed and delays the fix.

Why does it happen in adults?

• Scarring from infection. Repeated balanitis — inflammation of the glans and foreskin — leaves scar tissue that narrows the opening a little each time. The commonest acquired cause.
• Balanitis xerotica obliterans. A chronic skin condition producing a pale, thickened, inelastic ring at the tip. Does not respond to stretching and usually needs circumcision; the removed tissue is examined to confirm.
• Diabetes. High glucose in the urine and skin promotes yeast infection and scarring; new phimosis in a middle-aged man is a reason to check blood sugar on the ₩200,000 comprehensive blood test.
• Lifelong tightness. A foreskin that never fully retracted after puberty and was left. Often the mildest to treat.
• Forced retraction and small tears. Repeated splitting of a tight ring heals with more scar, not less. The cycle stops only when the ring is treated.

When do cream and stretching still work — the Busan urologist's test?

For grade 1–2 phimosis without scarring or a skin condition, a course of a topical corticosteroid applied to the tight ring twice daily for four to eight weeks, combined with gentle daily stretching — never forcing, never tearing — loosens the ring in a substantial proportion of men, and some stay loose. The urologist at our Busan clinic prescribes it, filled at a pharmacy, and reviews by message with a photograph at six weeks. It is worth trying first when the grade allows, because it is inexpensive and reversible. It is not worth trying when the ring is scarred, pale and inelastic, when the grade is 3–4, or when infections keep recurring — in those men it postpones the answer by two months and the ring is no looser at the end.

The two emergencies

Paraphimosis is a tight foreskin that has been retracted behind the glans and cannot be returned: the ring traps the glans, which swells, and within hours the swelling makes it harder still. It is painful, it is an emergency, and it is treated by reducing the foreskin under local anaesthesia — the same day, at our clinic during opening hours or at an emergency department outside them. Circumcision follows once the swelling has settled, because it will recur. Acute urinary retention from a pinhole phimosis — unable to pass urine, bladder distending — is the other, and is treated the same way: relieve first, circumcise later. When is circumcision the fix at our Busan clinic?

Grade 3–4 phimosis; a scarred or inelastic ring; balanitis xerotica obliterans; recurrent infections despite cream; a paraphimosis episode; and any grade in a man who has tried cream honestly and found it insufficient. In these men laser circumcision at ₩800,000 removes the ring with the foreskin, ends the cycle of splitting and scarring, and is performed under local anaesthesia in the clinic's own procedure room, same-day when the clinic knew in advance and there is no active infection. Alternatives to full circumcision

Two exist and both have narrow uses. A dorsal slit — a single cut through the tight ring, performed at our Busan clinic when needed — on the top surface — relieves the constriction quickly and is used in emergencies or in men unfit for circumcision; the result is functional but not cosmetic, and most men have a circumcision later. A preputioplasty — widening the ring with a small plastic-surgery technique while keeping the foreskin — suits a minority of men with a short, unscarred ring who strongly want to keep the foreskin; it does not suit scarred rings or skin conditions and has a real recurrence rate. The urologist will say which, if either, applies to you; for most adult phimosis, circumcision is the durable answer and the alternatives are the exception.

What treatment cannot promise

Three honest limits. Cream loosens a proportion of mild rings and none of the scarred ones, and no one can tell which you have until six weeks in. Circumcision leaves a fine scar and a permanently exposed glans, which most men adjust to within weeks and a few find changes sensation early on — said before surgery, not after. And where the underlying cause is a skin condition, circumcision treats the foreskin but the condition may need ongoing care of the glans; the examined tissue tells the urologist, and he tells you.
