Day 0: the afternoon after a vasectomy in Busan

After a no-scalpel vasectomy at Urogyn Men's Clinic in Busan Seomyeon you walk out within minutes of the procedure, with the local anaesthetic still working. The afternoon is for sitting or lying down: supportive underwear on, a cool pack for twenty minutes at a time if advised, simple pain relief before the anaesthetic wears off, and nothing heavier than a walk to a convenience store. Mild aching and a dull tightness in the scrotum as the evening goes on are normal; sharp pain, spreading swelling or bleeding through the dressing are not.

Days 1–2: the quiet days

The first morning usually brings a bruise — yellow to purple, sometimes spreading toward the base of the penis or the thigh — and a firm, slightly tender spot at the puncture site. Both are normal. The puncture itself, having no stitches, is already closing. Keep the support on, keep walking short, shower from day one with the dressing off and the area patted dry, and avoid a bath or a pool until the puncture is fully closed. Desk work is realistic on day two for most men, and many international patients spend day one resting in Seomyeon and day two on a short-haul flight; the flying guide covers long-haul, where the clinic advises a second night.

Days 3–7: back to normal, mostly

• 1 Days 3–4. Aching fades to an occasional twinge. Bruising at its most colourful, then fading. Normal walking, normal desk work, light chores.
• 2 Days 5–7. Most men feel essentially normal. The firm spot at the site persists for weeks and is scar tissue, not a problem. Support can come off during the day if comfortable; many keep it for exercise.
• 3 End of week one. Heavy lifting, running, cycling and gym work resume from here if the site is comfortable — start gently. A long walk around Busan is fine.

The one thing that persists past the week is the firm nodule at the puncture site, sometimes on each side. It is the healed vas ends and the fascial closure — the five-layer technique — and it shrinks over months without needing anything.

Weeks 2–4: activity and sex

Sexual activity, including ejaculation, resumes once tenderness has settled — typically around a week, sometimes ten days — and the first few ejaculations may contain a little blood or be slightly discoloured, which is normal and stops on its own. You are not sterile yet: sperm already beyond the ligation point are cleared over the following weeks and ejaculations, which is why contraception continues until the semen test. Cycling and contact sports are the activities men most often restart too early; two weeks is the sensible minimum for both, and a saddle in particular should wait until the site is comfortable to sit on. Flying home after a vasectomy

Short-haul — Japan, China, within Korea — is fine the next day: taxi to the airport rather than the subway stairs, aisle seat, support on, no heavy bag. Long-haul — Europe, the Americas, Australia — the clinic advises two nights, because swelling is more likely to increase over eight to fourteen hours seated and a dressing cannot be checked; compression stockings and hourly walking then apply as after any surgery. Warning signs, plainly — and when to message the Busan clinic

• Rapidly increasing swelling with pain in the first 48 hours — a haematoma. Message the clinic with a photograph at once; come in during opening hours.
• Fever, spreading redness, warmth, or pus at the site — infection. Uncommon after no-scalpel; treated promptly with antibiotics.
• Severe testicular pain out of proportion to the procedure, especially with swelling of the testis itself — assessed the same day.
• Bleeding that soaks through a dressing rather than spotting it.
• Persistent dull ache beyond a few weeks — usually settles, occasionally needs review; mention it by message.

The semen test at three months

A vasectomy is complete when a semen analysis confirms it — after about three months and roughly twenty ejaculations, the sample should show no sperm, or under some guidelines only rare non-motile sperm. Until that result, contraception continues. The test can be done at any laboratory at home; send the report to the clinic and the urologist confirms the result in writing. A first sample that still shows sperm is not a failure; a repeat after more ejaculations usually clears, and the urologist advises.

Long-term: what changes and what does not

Nothing about hormones, libido, erections, orgasm or the sensation of ejaculation changes — the testes continue to produce testosterone and sperm, and only the transport of sperm is interrupted; ejaculate volume changes so little that most men cannot tell. The firm spot at the site fades over months. A small proportion of men report a persistent dull ache beyond three months — post-vasectomy pain — which usually settles and occasionally needs treatment; it is uncommon, the urologist discusses it before the procedure, and it is not a reason to avoid the operation for most men. Recanalization — the ends reconnecting — is rare with the five-layer technique and is what the semen test exists to catch. Vasectomy is treated as permanent; reversal is complex and not offered at Urogyn.
