Why are length and girth different questions?

The first thing our urologists do is ask which state bothers you and in whose eyes: flaccid appearance in a changing room is a length conversation; erect sensation for a partner is a girth conversation; a fat pad hiding the shaft is neither surgery yet. What does length surgery in Busan change — and what does it not?
The VY advancement flap releases the suspensory ligament that tethers the shaft to the pubic bone and advances the skin so the released portion stays outside the body. The gain is real and mostly visible in the flaccid state, and modestly in the erect state at some angles; what it does not do is add to the erectile tissue itself — the erect penis is not lengthened in the way the word suggests, and a urologist who says otherwise is not describing this operation. Erect angle can lower slightly after ligament release, which men who value a high angle should hear before deciding. The gain is permanent, and the procedure is not reversible in any meaningful sense.

What does girth change — and what does it not?
Girth adds circumference to the shaft, and in filler and fat it does so softly enough to be felt as tissue rather than an implant. It is the change partners notice; it is also the change that photographs better than length. It does not add length, it does not change the glans unless glans enhancement is added, and in filler and fat it is not permanent in the way a graft is. A surprising share of men who ask about lengthening are advised to have something else first, or nothing. A suprapubic fat pad can bury two or three centimetres of shaft that no ligament release will recover — the answer is buried penis correction at ₩2,500,000, or, for men whose weight is still moving, weight loss first and surgery later. A penoscrotal web shortens the visible shaft from below and is corrected with a small procedure. And some men measuring themselves against an inaccurate reference are within the normal range and told so, with a chart, at consult. Revealing what is there is cheaper, safer and often more satisfying than creating what is not.
If both, which comes first?
Two rules our urologists apply. First, the reversible before the permanent: if a man is unsure, filler at ₩1,700,000 answers the girth question in a day and can be undone; length surgery cannot. Second, the structural before the cosmetic: if a fat pad or web is hiding length, correct that before adding girth, because the proportions change and the girth plan changes with them. Recovery: which is harder, and what does the trip look like?
- 1 Girth filler. Same-day; one night in Busan; sexual activity waits about a week. The easiest recovery on either side of this page.
- 2 Length surgery. Surgical; three to five nights before flying; stretching or traction as instructed in the weeks after to keep the released length; sexual activity waits several weeks. Harder than filler, comparable to a graft.

Realistic gains, stated plainly by our Busan urologists
Length surgery reveals length; it does not create erectile tissue, and gains are measured in centimetres in the flaccid state and less when erect. Girth adds circumference in proportion to the volume placed, and filler and fat both settle somewhat after the first weeks. Neither changes the glans without a separate step, neither is invisible on close inspection, and neither will make a man who is within the normal range feel differently about a number if the number was never the problem. - Does length surgery affect erections or sensation? Erectile function and sensation are not changed by ligament release; erect angle may lower slightly, which is the trade-off to weigh. Girth procedures do not affect erection or sensation either when properly placed.
- Will I need traction or stretching after lengthening? Usually yes, for a period the urologist specifies, to keep the released length from re-tethering as it heals. It is part of the recovery plan you leave with, not an optional extra.
