VY flap or Z-plasty? Which penile skin procedure fits which defect in Busan.
Both are plastic-surgery techniques for a penis short of skin, both are done at Urogyn, Busan Seomyeon, and men ask for them by name after reading about "skin lengthening". But they solve different geometric problems: a VY flap brings new skin in to cover a shortage, a Z-plasty redistributes the skin already there to release a tight band. This guide explains which defect gets which.

Male Enhancement (Length) in Busan
The hub behind this comparison — VY advancement, buried penis correction, webbing release, Z-plasty and the length package in Seomyeon.
Read the length hub →- I was told I need "skin lengthening" — is that a VY or a Z?
- Can a Z-plasty replace skin that a circumcision took away?
- Will either of these make my penis longer?
- Why is one priced and the other quoted?
- Can both be done at the same time?
Each is answered below, the way the surgeon would answer it in the consultation room at our Seomyeon clinic.
Two different problems: not enough skin versus skin in the wrong place
A VY advancement flap is used when the penis genuinely lacks skin — after an over-aggressive circumcision, after removing injected material, or where a short penoscrotal skin bridge tethers the shaft. It advances neighbouring skin in a V-to-Y pattern to cover the shortage, and at Urogyn in Busan Seomyeon it is listed at ₩2,500,000. A Z-plasty is used when there is enough skin overall but a tight scar band pulls in one direction, bowing or shortening the erect penis; two triangular flaps are swapped to lengthen the band at the expense of width. It is quoted at consultation because the scar pattern decides the extent.
Men searching "skin lengthening surgery" usually have one of two histories: a circumcision that took too much, or a previous enlargement that left scar. The examination at our Seomyeon clinic sorts out which geometric problem they actually have, and that — not preference — decides between the VY flap and the Z-plasty.
Brings adjacent skin forward to cover a true shortage. For circumcision over-resection, post-removal defects, penoscrotal tethering.
Swaps two triangular flaps to lengthen a tight linear scar. For a single band causing bowing or tethering, enough skin overall.
What the examination at our Busan clinic actually measures
Three things decide the technique. First, the stretched penile length compared with the skin available: if the shaft skin cannot reach the stretched length without pulling the scrotum up or the glans down, skin is genuinely short and a flap is needed. Second, the direction of pull: a tethering that bows the erect penis toward one side, or a visible tight line, points to a scar band that a Z-plasty can release. Third, the location: a shortage on the underside at the penoscrotal junction is classic VY territory; a band across the shaft from an old incision is classic Z-plasty territory. Our length hub explains stretched length measurement, which underlies every procedure on that page.
The VY advancement flap in detail
A V-shaped incision is made in the skin adjacent to the defect — usually the scrotal or suprapubic skin — the flap is advanced into the gap, and the donor site is closed as a Y. The advanced skin keeps its own blood supply, so it heals reliably and matches in colour better than a graft from elsewhere. At our Seomyeon clinic it is a day procedure under sedation with local anaesthesia, about an hour, with the ₩2,500,000 published price covering surgery, anaesthesia and follow-up. It is often combined with buried penis correction or webbing release when those are the cause of the apparent shortage.
- 1 Best for. True skin shortage on the ventral side; tethered penoscrotal junction; defects after vaseline or filler removal.
- 2 Trade-off. A Y-shaped scar at the donor site; some scrotal skin may be brought onto the shaft, which looks slightly different.
- 3 Recovery. Desk work in 3–5 days; intercourse at about 4 weeks; scars mature over months.
The Z-plasty in detail
A Z-shaped incision is centred on the tight band, creating two triangular flaps that are lifted and transposed. The central limb of the Z — the scar — ends up rotated and lengthened by roughly half to three-quarters again, at the cost of width, which the shaft has to spare. No new skin is brought in; the existing skin is redistributed. It suits a single linear contracture, which is why it is the standard answer for a bowstring scar from a previous operation, and why it is the wrong answer when the whole circumference is short. It is quoted at consultation because a single Z and a series of Zs along a long scar are different operations. The Z-plasty page shows the geometry.

About the clinic →When both are used in the same operation
They are not rivals. A man whose circumcision removed too much skin and healed with a tight scar across the underside may need a Z-plasty to release the band and a VY flap to supply the missing skin — in the same sitting. A man who had injected material removed may have a defect that needs a flap and a contracted scar at the edge that needs a Z. At our Busan clinic the plan is drawn on the skin before anaesthesia and photographed, and you see it. Our correction after enlargement complications guide covers the sequence when skin shortage follows a removal.
What neither procedure does to erect length — said plainly
Neither a VY flap nor a Z-plasty lengthens the penis itself. They restore skin so that the length you already have can extend fully without pulling, and they relieve the bowing or tethering that a tight band causes — which can make the erect penis look and feel longer than before, because it was being held back. Men expecting a longer shaft are directed to the length hub, where ligament release and the length package are explained with their own realistic limits. Our length vs girth guide covers that decision.
Price: what is published and what is quoted
The VY flap has a published price of ₩2,500,000 at Urogyn, covering surgery, sedation with local anaesthesia and follow-up. Z-plasty is quoted at consultation because the extent varies from a single Z to a chain. When the two are combined, the figure is the sum of the published and quoted parts, confirmed in writing before booking. Pre-operative blood tests are separate and listed on the full price list. There is no foreigner surcharge.
Published. Day procedure, sedation with local, follow-up included.
Single Z versus multiple; set after examination and given in writing.
What to bring from home so the Busan visit decides quickly
Skin reconstruction depends heavily on what was done before, so the single most useful thing to send is the operation note from the previous surgery — circumcision, enlargement, removal — with its date, even if it is brief or in another language. Second, photographs: flaccid and erect, from the side and from below, in good light, showing where the skin pulls. Third, a note of any wound problems or infections since. With these our surgeon can usually say by message whether a flap, a Z-plasty or both is likely, and the measurement on arrival in Busan confirms rather than discovers. Men whose previous surgery was abroad and undocumented are not turned away; the examination simply carries more of the weight. Our what to bring guide covers the practical list.
Fitting skin surgery into a trip to Busan
Both are day procedures. Plan five to seven nights in Seomyeon: consultation and measurement on arrival, surgery within a day or two, a dressing check at about day three and a review before flying. Intercourse waits about four weeks, which is usually after you are home; we follow scars by photo. See flying after surgery for timing and the Seomyeon hotel guide for the recovery week.
How it runs at our Busan Seomyeon clinic — and your next step
Message us with what was done before and when, where the skin feels tight, and photos from the side and below, erect if possible. We reply in writing with which problem it looks like, which procedure or combination we would plan, and the figure. Nothing is booked until you confirm and no deposit is taken. The clinic is one minute from Seomyeon Station exit 7; the contact page has directions.
- Will scrotal skin on the shaft look different? Slightly — it is a little darker and more textured. We keep the flap as low on the shaft as the defect allows, and over a year the difference softens. We show you where it will sit before surgery.
- How long before the scars settle? Scars are red and firm for about three months and continue to soften and fade for a year. Silicone sheeting and sun avoidance help; we give you the protocol in writing to continue at home.
