Local, sedation or general? How anaesthesia is chosen for men's surgery in Busan.
Men planning a procedure abroad ask about the surgeon and the price and rarely about the anaesthesia — until the night before. This guide from Urogyn, Busan Seomyeon, explains which operation gets which anaesthetic and why, what the pre-operative checks are, the fasting rules, and what to tell us about your medication before you travel.

Urogyn Men's Clinic, Busan Seomyeon
The clinic behind this guide — three board-certified urologists, an on-site operating theatre, and English consultation one minute from Seomyeon Station.
About the clinic →- Will I be awake, and will I feel it?
- Can I choose to be fully asleep for a smaller procedure?
- I take a blood thinner — do I stop it?
- How long do I have to fast, and can I have coffee?
- Can I travel alone after the procedure?
Each is answered below, the way the surgeon and anaesthetist go through it with you before any procedure at our Seomyeon clinic.
The three options in one view — anaesthesia at our Busan clinic
At Urogyn in Busan Seomyeon, men's procedures use one of three approaches. Local anaesthesia alone for short, superficial procedures — filler, nerve blocks, wart removal, vasectomy. Local with intravenous sedation for procedures of thirty to ninety minutes where you should be comfortable and unaware but do not need to be fully asleep — UroLift and Rezum, circumcision, gynecomastia surgery, Peyronie's plication, skin flaps. General anaesthesia with an anaesthetist for the longer or deeper operations — penile implants, grafting, reconstruction. The choice is set by the operation and your medical history, not by preference, and is written into your plan before you book.
The sections below explain what each involves, what you will feel, and the preparation. The one rule that applies to everything beyond local alone: an adult must be able to collect you or you must return to a hotel within walking distance, because you will not drive, travel alone on the subway, or make decisions for the rest of that day.
Numbing injection at the site. You feel pressure, not pain. Walk out; eat normally; no fasting.
IV medication so you doze; local at the site. 6-hour fast. Rest 1–2 hours after; back to the hotel, not alone.
Anaesthetist present, airway managed. 8-hour fast. Overnight stay usual. Pre-op blood tests required.
Which operation gets which — the list
The pairing is driven by how long the operation takes, how deep it goes, and how still you need to be. A ten-minute filler injection needs only a numb surface; a two-hour graft operation needs a man who does not move and does not remember. Between them sits the large group of procedures done under sedation, where you are drowsy and comfortable, the surgeon has local anaesthesia at the site, and you wake with little memory of the hour.
- 1 Local only. Penile filler, frenular and triple nerve block, no-scalpel vasectomy, wart removal, prostate biopsy referral checks, injections for Peyronie's.
- 2 Local + sedation. UroLift, Rezum, Progator, adult circumcision, gynecomastia surgery, Peyronie's plication, VY flap and Z-plasty, buried penis correction, fat graft, dermal graft (shaft), testicular prosthesis.
- 3 General. Penile implants, Peyronie's grafting, scrotal flap reconstruction, extensive vaseline removal, combined length packages.
Local anaesthesia: what it actually feels like
The honest description is a sharp sting for a few seconds as the anaesthetic is injected, then pressure and movement without pain for the duration. For penile procedures the injection is placed at the base of the penis — a penile block — and numbs the whole shaft and glans within minutes; most men describe the injection as the only uncomfortable part. You are awake, can talk to the surgeon, and walk out afterwards. The numbness lasts three to four hours; take the first painkiller before it wears off. No fasting is required and you can travel alone. The vasectomy page describes the technique used at our Seomyeon clinic to make the injection itself nearly painless.
Sedation: the middle option most men end up with
Intravenous sedation uses short-acting medication through a cannula in the hand or arm. Within a minute or two you are drowsy and unbothered; local anaesthesia is then placed at the site, which you barely register, and the procedure proceeds with you dozing. You breathe for yourself and are monitored throughout — oxygen, heart rate, blood pressure — by a nurse dedicated to that role. Most men have little or no memory of the hour. You wake quickly, rest for one to two hours in the recovery room at our Busan clinic, and return to the hotel accompanied. For the rest of the day you must not drive, drink alcohol, sign documents or travel alone; by the next morning you are clear. Fasting is six hours for food and two for clear fluids. Pre-operative checks are a health questionnaire and, for men over fifty or with medical conditions, a blood test.

About the clinic →General anaesthesia: when it is used and what it adds
General anaesthesia is reserved for the longer and deeper operations — implants, grafts, reconstruction — where complete stillness and full control of pain and breathing matter. An anaesthetist is present throughout, your airway is supported, and you wake in the recovery room with no memory of the operation. An overnight stay is usual, both for the operation's own needs and so that the first hours after general anaesthesia are supervised. Pre-operative requirements are stricter: a comprehensive blood test (₩200,000, or ₩100,000 with other tests), an ECG for men over fifty or with heart history, an eight-hour fast, and a full medication review. Our penile implant cost guide explains why the anaesthetist and the stay are included in the implant figure.
Medication and history: what to tell us before you travel
Three groups of medication change the plan and must be declared when you first message, not on the morning. Blood thinners — warfarin, DOACs, clopidogrel, and regular aspirin — are paused or continued according to a written plan agreed with your own doctor; stopping them yourself is dangerous and not stopping them when instructed causes bleeding. Diabetes medication interacts with fasting; we adjust the morning dose. Erectile dysfunction medication is stopped for a set period before penile surgery. Beyond medication, tell us about previous anaesthetic problems, sleep apnoea, heart or lung disease, allergies, and whether you smoke, because smoking affects both anaesthesia and healing — our safety standards page covers the clinic's protocols.
- 1 Always declare. Blood thinners, diabetes drugs, ED medication, steroids, herbal supplements (several affect bleeding).
- 2 Always mention. Previous anaesthetic reactions, sleep apnoea, heart or lung disease, allergies, smoking, alcohol intake.
- 3 Bring. A current medication list with doses, and any recent blood results or ECG.
Fasting, the morning of surgery, and the hours after
For local only: eat and drink normally; a light meal beforehand prevents light-headedness. For sedation: nothing to eat for six hours, clear fluids until two hours before, usual medication with a sip of water unless told otherwise. For general: nothing to eat for eight hours, clear fluids until two hours before. Arrive in loose clothing, without contact lenses or jewellery, with your companion or your hotel address. After sedation or general, the first hours bring drowsiness and sometimes nausea, both short-lived; eat lightly that evening. Alcohol is avoided for twenty-four hours after sedation and forty-eight after general. Our first visit guide describes the day in order.
Anaesthesia and travel: planning the nights around it
Local-only procedures need no extra nights; many men fly the next day. Sedation procedures need the night of surgery in Seomyeon and a check the next morning or day after. General anaesthesia means the overnight stay at the clinic and several more nights nearby before flying. A companion is strongly recommended for sedation and general, and required for general if you are staying in a hotel rather than with us overnight. Our flying after surgery guide covers the procedure-specific flight timing, and the hotel guide lists walking-distance options.
How it runs at our Busan Seomyeon clinic — and your next step
Message us with the procedure you are considering, your age, your medical conditions, every medication and supplement you take, and whether someone is travelling with you. We reply in writing with the anaesthetic plan for that procedure, the pre-operative checks you will need, the fasting instructions and the medication plan to agree with your own doctor. Nothing is booked until you confirm and no deposit is taken. The clinic is one minute from Seomyeon Station exit 7; the contact page has directions.
- Is sedation the same as general anaesthesia? No. Under sedation you breathe for yourself, respond if spoken to, and recover within an hour or two; under general your breathing is supported by an anaesthetist and recovery takes longer. Sedation is lighter, with fewer side effects and no overnight stay.
- What if I wake up during sedation? You may drift in and out — that is how sedation works, and the local anaesthesia means it is not painful. Depth is adjusted continuously by the monitoring nurse; genuinely distressing awareness is not expected.
