Glans enlargement: filler or dermal graft? How Urogyn Busan decides.

Glans enlargement is a proportion procedure, not a size procedure, and the two ways of doing it suit two different men. Hyaluronic filler is reversible and done in minutes; a dermal graft is permanent and is almost always done alongside a shaft dermal graft. This guide from Urogyn, Busan Seomyeon, explains which is right for whom — and when neither is.

What readers ask before they message us
  1. If I am only doing the glans, which should I choose?
  2. Does glans enlargement help with premature ejaculation?
  3. If I have a shaft dermal graft, do I have to do the glans too?
  4. Will filler in the glans change how it feels?
  5. How often would I need to come back to Busan for filler?

Each is answered below, the way the surgeon would answer it in the consultation room at our Seomyeon clinic.

Why glans enlargement is about proportion, not size — at our Busan clinic

The glans is enlarged for one of two reasons: to restore proportion after the shaft has been thickened, so the head is not left smaller than the body; or to fill a glans that was always small or pointed relative to the shaft. At Urogyn in Busan Seomyeon the two methods are hyaluronic acid filler — reversible, done under local anaesthesia in about twenty minutes, quoted at consultation with the shaft filler prices as the reference — and a dermal graft placed beneath the glans skin, which is permanent and is almost always combined with a shaft dermal graft from ₩3,900,000.

Because it is a proportion procedure, the first question is never "how much bigger" but "bigger relative to what". A man planning shaft enlargement decides the glans question as part of that plan; a man with no shaft work planned is asking a different question with a more conservative answer. The glans enlargement page and the girth hub set out both.

Hyaluronic filler
Reversible

Local anaesthesia, about 20 minutes, back to work the same day. Lasts 12–18 months; dissolvable with hyaluronidase.

Dermal graft
Permanent

Placed beneath glans skin, usually with a shaft dermal graft. Surgical recovery; not reversible.

First-time glans enlargement: why filler comes first

A man who has never had any enlargement and wants a fuller glans is offered filler at our Seomyeon clinic, for three reasons. It is reversible — if the look or the feel is not what he expected, hyaluronidase dissolves it within days. It is conservative — a small volume shows what proportion looks like before anything permanent is done. And it is proportionate to the problem — a glans that was always slightly small needs a modest change, not a graft. The filler page explains the products used, all MFDS-approved, and our guide on verifying MFDS approval shows how to check.

The seashell trap. Enlarging the glans too much, or enlarging it alone when the shaft is slim, produces a top-heavy shape men regret. Filler lets that be corrected; a graft does not.

Paired with a shaft dermal graft: why the graft wins here

When a man is having a shaft dermal graft — permanent girth from ₩3,900,000 — the glans is usually enlarged in the same operation, and a dermal graft under the glans is the natural match. The tissue is the same, the operation is already surgical, the result is permanent on both parts so proportion stays constant, and there is no need to return every year to top up the head while the shaft remains unchanged. Filler can still be used on the glans alongside a shaft graft if a man wants reversibility there, but it introduces a maintenance cycle for one part and not the other. The dermal graft page describes the combined procedure.

  • 1 Shaft dermal graft + glans dermal graft. One operation, one recovery, permanent proportion. The usual combination at Urogyn.
  • 2 Shaft dermal graft + glans filler. Permanent shaft, adjustable head. For men unsure how much glans they want.
  • 3 Shaft filler + glans filler. Everything reversible. The first-time, test-the-proportion option.

Feel, sensation and what each does to the glans surface

Hyaluronic filler sits in a plane beneath the skin and feels soft, close to natural glans tissue when the correct product and volume are used; the glans surface and its sensation are unchanged because nothing is placed in the sensory layer. A dermal graft is firmer in the first months and softens as it integrates, and because it is placed surgically there is a short period of altered sensation while the skin heals. Neither is intended to reduce sensitivity — men asking about glans enlargement to treat premature ejaculation are directed to the PE hub, where the sensitivity test and the frenular and triple nerve blocks are the correct tools.

About the clinic →

How long each lasts, and what maintenance means

Filler in the glans lasts about twelve to eighteen months, somewhat less than in the shaft because the tissue is more mobile. Maintenance means a smaller top-up visit, which international patients often combine with a return trip or arrange nearer home with a clinic that uses the same product. A dermal graft is permanent; it settles over six months and does not need maintenance, which is its whole argument. The choice between them is partly a choice about whether you want to think about this again. Our filler vs fat vs dermal guide covers the same question for the shaft.

Who should not have glans enlargement at all

A man with an active infection or inflammation of the glans or foreskin, a man with uncorrected phimosis — see adult circumcision — and a man with previously injected material of unknown type are not treated until those are resolved. A man whose motivation is premature ejaculation is redirected, as above. And a man asking for a dramatic change in a glans that is already proportionate is told, in writing, that the result would look worse. At our Busan clinic that conversation happens before any booking, and costs nothing.

Price: what is published and what is quoted

Glans filler is quoted at consultation, with the published shaft filler prices (₩1,700,000 for 2–4 cc; ₩2,200,000 for the 15 cc base product) as the reference point; glans volumes are small. A glans dermal graft is quoted as part of the combined shaft-and-glans procedure, with the shaft dermal graft published from ₩3,900,000. Both figures are given in writing before booking and nothing is added on the day. The full price list has every item, and the enlargement cost guide explains what moves the figures.

Glans filler
Quoted · small volume

Reference: shaft filler 2–4 cc ₩1,700,000. Local anaesthesia, same-day return to work.

Shaft + glans dermal graft
from ₩3,900,000

Combined operation; glans component quoted with the shaft figure. Permanent.

What to bring from home so the Busan visit decides quickly

For a glans question the useful history is short: whether you have ever had any injection into the penis — and if so, what product, where and when — whether shaft enlargement is part of the current plan, and two photographs, flaccid and erect, from the side. Previous injected material of unknown type is the one thing that changes the plan entirely, because filler cannot be placed over unknown material and a graft cannot be placed beside it; our correction after complications guide explains the sequence in that case. Men planning a shaft dermal graft at the same visit should allow for the combined procedure's week in Seomyeon rather than the single filler day. Our what to bring guide covers the practical list, and the enlargement recovery guide covers the weeks afterwards.

If you are uncircumcised. Tell us. A foreskin that does not retract fully makes glans filler placement and assessment unreliable, and the circumcision question comes first.

How it runs at our Busan Seomyeon clinic — and your next step

Message us with whether you are planning shaft enlargement as well, whether you have had any previous injections, and what bothers you about the current proportion. We reply in writing with which method we would recommend and why, and the figure. Nothing is booked until you confirm and no deposit is taken. The clinic is one minute from Seomyeon Station exit 7; the contact page has directions and the girth hub has the full enlargement map.

Two more questions we hear on the day
  1. Can filler be removed if I do not like it? Yes. Hyaluronidase dissolves hyaluronic filler within days, at ₩1,500,000. This is the whole reason filler is offered first to men who have never had enlargement.
  2. Can a glans dermal graft be done without a shaft graft? Technically yes, but it is rarely advised. A permanent change to the head alone, with no shaft work, is the shape men most often regret. We would discuss filler first.

Glans enlargement filler vs dermal graft — FAQ

For a first-time, glans-only change, hyaluronic filler: reversible, twenty minutes under local anaesthesia, dissolvable if the proportion is wrong. Alongside a shaft dermal graft, a glans dermal graft: permanent, one operation, proportion that stays constant. Urogyn in Busan recommends by situation, not preference.
No. Neither method is designed to reduce sensitivity. Men whose concern is PE are directed to the sensitivity test and the frenular or triple nerve block, which are the correct tools.
About twelve to eighteen months, slightly less than shaft filler because the tissue is more mobile. A top-up is a smaller visit; a dermal graft is permanent and needs no maintenance.
Filler sits beneath the skin and, with the right product and volume, feels soft and close to natural. The surface and its sensation are unchanged. A dermal graft is firmer at first and softens over months.
Glans filler is quoted at consultation with the published shaft filler prices (₩1,700,000 for 2–4 cc) as the reference. A glans dermal graft is quoted with the shaft dermal graft, published from ₩3,900,000. Figures are confirmed in writing before booking.
No — it is a same-day procedure under local anaesthesia and you can fly the next day. A combined shaft-and-glans dermal graft needs about a week in Seomyeon for the first dressing checks.
Three things: a dramatic size change, any effect on premature ejaculation, and a glans dermal graft that can be undone. Realistic expectations are a proportionate, natural-feeling head matched to the shaft, placed conservatively.
